SAU clinical paperwork: three forms and a four-week deadline

The 2023-24 Graduate Catalog names three kinds of clinical paperwork, the clinical agreement, affiliation and preceptor verification forms, and says they are due at least four weeks before a clinical course. SAU does not publish the forms or their exact titles, so get the current versions from your program and plan around that four-week floor.

What the catalog actually says

The rule is one sentence in the post-master's certificate section of the 2023-24 catalog: students submit all clinical agreement, affiliation and preceptor verification forms "at least four (4) weeks prior" to any clinical course. That is the whole published text. No SAU page lists the forms by title, shows a sample or says where they are submitted.

Because the sentence sits in the certificate section, MSN and RN-to-MSN students sometimes wonder whether it covers them. Nothing SAU currently publishes in readable form answers that. The newer catalogs sit behind a human check, so the safe course is to ask your program for your deadline and treat four weeks as the latest possible date, not the target.

Catalog wording and course numbers may have changed since 2023-24; your advisor can tell you which catalog governs your cohort.

The three forms, as far as their names tell us

SAU hasn't published the forms, so what follows is a reading of the catalog's phrase, not a description of SAU's documents. Get the real versions from your program before you ask anyone to sign.

  • Clinical agreement. Most likely the arrangement for the rotation itself: who, where, which course and when. Expect it to need your preceptor's signature and possibly the site's.
  • Affiliation. Tied to the relationship between SAU and the clinical site. Where a site already has an agreement this step may be short; where it doesn't, it can be the slowest part. See affiliation agreements.
  • Preceptor verification. Most likely confirms the preceptor's credentials against SAU's rules for your track, which are set out on preceptor requirements.

Never rebuild a form from memory or borrow a template from another school. Use what your program sends, and if a version looks out of date, ask before you fill it in.

Counting back from your course start

Start from the date your clinical course opens and count backwards, not forwards from when you feel ready. Four weeks before that date is the catalog's floor. Ahead of it you need a preceptor who has said yes, a site that has cleared any agreement step, and compliance records that are current, because faculty look at all of it together.

SAU runs seven-week courses with a week off between them, so four weeks is a little over half of one course. If a didactic course is running while the forms are due, aim to finish the packet during the break week or earlier, not in the last days of a heavy assignment week. What to gather before you ask for signatures

SAU does not publish a packet checklist for preceptors, but its published rules show what faculty will need to see. Collect these before the first form goes out, so each signature request is one visit rather than three.

A practical gathering list

  • The preceptor's full name, credential (NP, physician or PA; for NUR 780B, possibly a master's-prepared nurse) and specialty.
  • License number and state, plus board certification details for an NP.
  • A current CV, if your program asks for one.
  • The site's legal name, street address and a contact for agreements.
  • The course number and the dates your rotation will run.
  • The hours you plan to complete there that term.
  • Your own compliance documents, valid through the end of the rotation.

The staff checklist mentioned in SAU's clinicals post may list more than this. Ask for it early, since it is the nearest thing to an official list.

Your compliance file sits beside the forms

Faculty review your records along with the preceptor paperwork. The 2023-24 catalog lists an unrestricted RN license in your state of practice, professional liability insurance for the whole enrollment, a federal or international background check, a drug test, CPR/BLS, TB testing, required immunizations and HIPAA training, with the background check and drug test repeated yearly. You pay for all of it and keep the records current with the nursing department.

SAU warns that noncompliance "may result in program delay or academic withdrawal." A flawless preceptor packet does little if your TB result lapses the week before the rotation. The full list and its timing are on clinical compliance.

Check expiry dates against the last day of the rotation, not the first. A CPR card or TB result that runs out mid-rotation can pause your hours just as they are building.

Questions Spring Arbor students ask

Where do I get SAU's clinical agreement and preceptor verification forms?

From your program. SAU does not post the forms publicly, and no public page names the system they sit in. SAU's clinicals post says staff and faculty will give you a list of required documents before your first clinicals, so ask your course faculty or program director for the current set and where to send it.

Does the four-week rule apply to MSN students or only to certificates?

The only published version sits in the post-master's certificate section of the 2023-24 catalog, and no MSN-specific wording can currently be read. Ask your program for your deadline in writing. Until you hear otherwise, plan as if four weeks before the course is the latest you can submit.

What happens if my SAU clinical forms are late?

SAU doesn't publish a specific consequence for late forms. The catalog does warn that unmet requirements may delay your program, and a clinical course cannot go ahead without an approved preceptor and site. If you can see you will miss the date, tell your course faculty before it passes, not afterward.

Do I need new paperwork for each semester of a multi-semester practicum?

SAU says preceptors and sites must be approved for every rotation, so expect fresh paperwork whenever the preceptor, the site or the rotation changes. Whether a continuing preceptor needs a full new packet each semester is not published. Ask your program, and keep copies of everything you sent the first time.

Can my preceptor complete the forms after my rotation starts?

No SAU page allows it. The catalog puts the forms at least four weeks before the clinical course, and faculty approve the preceptor before the rotation. Starting first and catching up on forms later risks hours that do not count, so collect signatures well before your first day.
